Tic Disorders & Tourette Syndrome Assessment

Specialist assessment and treatment by Consultant Neuropsychiatrist Dr Saidi. Tics are sudden, repetitive movements or sounds that are difficult to control. While many childhood tics improve over time, some persist into adulthood or significantly affect daily life. An accurate assessment can help identify the type of tic disorder, rule out other conditions, and discuss appropriate treatment options.

Understanding tics

What are tic disorders?

Tic disorders are neurological conditions characterised by involuntary, rapid, recurrent movements (motor tics) and/or vocal sounds (vocal tics). These movements are not performed intentionally and may temporarily be suppressed, although doing so often causes increasing internal discomfort before the tic occurs.

Tics commonly begin during childhood, usually between the ages of 5 and 10 years, and may change in frequency or type over time. Symptoms often become worse during periods of stress, anxiety, excitement or fatigue and may improve when a person is relaxed or concentrating on an enjoyable activity.

Many people with tic disorders also experience other neurodevelopmental or mental health conditions such as ADHD, obsessive-compulsive disorder (OCD), anxiety or autism spectrum disorder.

Diagnostic criteria

What is Tourette syndrome?

Tourette syndrome is a neurological condition that causes both:

  1. Multiple motor tics

More than one involuntary, repetitive movement.

  2. At least one vocal tic

At least one involuntary sound, word or noise.

Both must have been present for at least one year

These do not necessarily occur at the same time, but have been present for at least one year, with onset before the age of 18.

Contrary to popular belief, only a small proportion of people with Tourette syndrome experience involuntary swearing (coprolalia). Most individuals have other types of motor and vocal tics.

The appointment

What an assessment with Dr Saidi involves

An assessment is a single, unhurried appointment. Dr Saidi takes a full history, looks at the tics themselves and at anything occurring alongside them, and talks through what the findings mean for you. You leave with a diagnostic opinion where one applies, a written report, and a plan for what happens next.

The assessment

During your appointment

Dr Saidi will carry out a comprehensive clinical assessment, which may include:

  • Detailed medical history
  • Developmental history
  • Description of current tics
  • Review of previous diagnoses or treatments
  • Assessment for ADHD, OCD, anxiety or autism where appropriate
  • Discussion about how symptoms affect everyday life

Following the assessment

You will receive:

  • A diagnostic opinion where appropriate
  • Individualised treatment recommendations
  • A comprehensive clinical report
  • Advice regarding ongoing management and follow-up care
Management

Treatment options

Treatment depends on the type of tic disorder, symptom severity and the impact on daily life. Management may include:

  • Education about tic disorders
  • Advice on reducing triggers
  • Behavioural therapy, such as Comprehensive Behavioural Intervention for Tics (CBIT) or Habit Reversal Therapy (HRT)
  • Medication where clinically appropriate
  • Management of co-existing conditions such as ADHD or OCD

Not everyone with tics requires medication

Many people benefit from education, behavioural approaches and monitoring alone.
